Submit a Claim - Claim Instructions

If you are a part of the Settlement Class, (see FAQ 6) and you would like to receive a payment from the Cash Settlement Fund, you must file a Claim Form on or before June 19, 2017. There are several methods in which to file your Claim Form and they are described below and in FAQ 9.


How to File Online

After clicking the button below, you will be taken to a page to log in with information from your postcard that you received from the Settlement Administrator. If you did not receive a postcard or lost yours, please contact the Settlement Administrator at 1-844-616-6615.

Afterwards, you will be asked to provide updated address information, if applicable, and your digital signature. Claim Forms must be submitted online no later than 11:59 p.m. PST on June 19, 2017.

How to File by Email or Fax

You may also email or fax your completed Claim Form that was sent to you. You may also download a copy here. Please read the Claim Form carefully and provide all the information required. Then submit it via email to info@FirstMeritOverdraftSettlement.com or fax it to 1-844-603-5052. Claim Forms submitted by email or fax must be received no later than 11:59 p.m. PST on June 19, 2017.

You may also mail back your completed Claim Form that was sent to you. You may also download a copy here. Please read the Claim Form carefully and provide all the information required. Claim Forms submitted by mail must be postmarked on or before June 19, 2017 to:
